Emotional Service Animal (ESA) Letters
Roots Counseling has a specific process for individuals to be able to obtain an ESA letter. We have worked hard this process as simple as possible. To obtain an ESA letter, we need your first and last name, email, and phone number. After this, we will email you an in depth questionnaire/assessment to fill out. We charge a $125 fee for providing ESA letter services. Once the fee is paid and the questionnaire/assessment is complete, we will then write up and send you the requested ESA letter.
